It is a two sided card that contains a photo, name, address, DOB, height, weight, hair color, eye color, identifying marks, two thumbprints, medical information, parents names and telephone numbers and a parental release signature.

Our C.O.P.S. (Child Official Personal Safety) card is designed to be carried by a parent, guardian, babysitter, teacher on a field trip or the child.

Size - Size of your standard business card.

Media Type - Business card paper or 60 lb. to 80 lb. white card stock paper.

Printing Equipment - Standard desktop color inkjet printer or color laser printer.
